Every team is required to check in 1 hour before their first game. You will check in at the complex of your first game.

 

 

-         You must have an original roster and 4 copies with players numbers

 

-         You must have a signed, laminated player pass for every player and coach participating on your team in the tournament

 

-         If a team is using guest players then the proper FYSA “EVENT ROSTER” or guest player form must be completely filled out. 

 

-         Every player that is participating in the tournament must have a notarized medical release form. You will be required to show them at check in. A player will not be permitted to participate in the tournament if they do not have a notarized medical release.

 

-         4 stamped, approved rosters

-         player passes

-         copy of any guest player forms

-         medical release forms

 

1.      Give stamped approved roster to official

2.      Officials will check players for safe and proper equipment

3.      Coaches will initial score on game report after each game

4.      If you fail to initial the game report the tournament will not be held responsible for incorrect score reporting.

5.      The referee will not take you player passes
